*If you do not report to Central Check in, your team will forfeit all games won; or lose
  participation points if you have not won a game.

All teams MUST report to Central Check-In.  If this is your first qualifier of the year your entire team must be present.

You DO NOT have to check in players who have already been approved.  Only roster player additions need to be present during mandatory coaches’ check-in.

*Bring the team book. The book MUST have the following items:

1. Official USSSA Team Roster (with completed signatures of players/parents)
2. Birth Certificate of each player
3. Current picture of each player
4. Current school year report card

Coaches Association

It is mandatory that all coaches and personnel that plan on sitting on the players’ bench become members of the American Coaches Association (ACA) ONLY 3 PER TEAM.

The purpose of the ACA is to help maintain the highest standards in the coaching profession and to further the best interest of sports for its memberships as well as the players and all that participate.

Requirements of Membership:
1. Submit application along with photo and membership fee ($25)
2. Forms can be accessed at www.flahoop.org/coachesassociation.com or at check in
3. If you have joined then bring your picture id or proof of the application sent.
4. If you have not submitted the application, please fill out the form before Friday and bring it with you along with your current photo and the $25 fee.

(SAC) Sagemont School Upper Campus – Sagemont Athletic Center - 2585 West Glades Circle, Weston, FL 33314

From I-595 west take 75 north toward Naples.  You will get off at exit 22 (Glades Parkway), make a left on to Glades Pkwy and then make the 1st right which is Glades Circle (Publix on right).  Sagemont School is on the west side of the road and the Gym is located to the left side of the school.  (Please do not park in church parking lot on Sunday mornings)

(Dome) Sagemont School Lower Campus – The Dome – 1570 Sagemont Way, Weston FL 33326

From I-595 west take 75 south toward Miami.  Get off at exit 15 (Royal Palm Blvd.).  Stay on Royal Palm to Bonaventure Blvd and make a right.  Go to Three Village Road (traffic light) and make a left and then a right at Sagemont Way.  The Dome is on the right next to the pool.   

(GD) Gloria Dei Lutheran Church - 7601 S.W. 39th Street, Davie, FL 33328

From I-595 exit University South.  You will be on University for about 3-5 minutes.   Turn left on SW 39th.  Church is located 100 meters to your left. Gym is located in the rear of the parking lot.

(NOVA)Nova High School – 3600 College Ave, Davie FL. 33314

From I-595 exit at Davie Rd Extension and go South to SW 39th street.  Go up the road about quarter of a mile and the entrance to the gym will be on your right across from the Dairy Queen. 

(STR) Stranahan High School 1800 SW 5th Pl Fort Lauderdale, FL 33312

From I-95 Exit Broward Blvd East, Turn right at SW 18th Ave, Destination will be on the left.

(UN)University School – 3301 College Avenue, Sonken Bldg; Fort Lauderdale FL. 33314

From I-595 exit at University Drive, go south on University to SW 42nd Dr. and make a left (Bank of America is on the North East corner) go to the stop sign and make a left before the pool.  If there is construction you will have to park off of 42nd and walk up to the gym which is north of the pool.

(CB)Cypress Bay High School – 18600 Vista Park Blvd, Weston FL. 33326

From I-595 go west to 75 South toward Miami.  Get off at exit 15 (Royal Palm Blvd).  Stay on Royal Palm to Bonaventure Blvd and make a left.  Take Bonaventure south for approximately 2 miles to Vista Park Blvd and make a right and the school is on the left.

Take I-95 toward Deerfield Beach to exit 41 for FL-869/SW 10th St. turn right (east) at SW 10th/FL-869 (signs for Deerfield) Turn left at SE 6th Ave.  Zion on the left

(HC)Highlands Christian School 501 N.E. 48th Street Deerfield Beach, FL 33064

Take I-95 toward Deerfield Beach to exit 39 for Sample Rd/FL-834, keep right at the fork, follow signs for Lighthouse Point and merge onto NE 36thSt/FL-834/E Sample Rd.  Turn left at FL-811.  Turn left at NE 48th St and the school will be on the right.

(BRT) Wayne Barton Study Center 269 N.E. 14th Street Boca Raton, FL 33432

Take I-95 toward Boca Raton exit 45 for Glades Rd/FL-808, keep right at fork and follow signs for Boca Raton.  Merge onto FL-808/Glades Rd.  Turn left at NE 1st Ave/FL-811.  Turn right at NE 15th Terrace and turn right at NE 2nd Ct.  NE 2nd Ct turns left and becomes NE 14th Str. Barton is on the left.
